To add new providers to your existing Majoris contract so they will also be authorized to treat Majoris enrolled injured workers, you will need to complete and submit a credentialing application for each provider, along with the supporting documentation requested on the application. Each application must be reviewed and approved by the Majoris Medical Director. Please be advised that until you receive written notice that the new provider has been approved and added to panel, providers are not authorized to treat Majoris enrolled injured workers unless given express permission for the specific claim.
